The top of the report card contains places for traditional letter or numerical grades.

The bottom of the report card contains space for indicators of student achievement on performance standards. Beside each subject is a list of numbers ranging from 1 to 15 corresponding to the respective performance standard for each subject. Under each reported standard are places to indicate the student's status toward reaching the standard: ACH for if the student achieves the standard; IP if the student is in progress toward achievement; and X if the student does not achieve the standard.
